Referring Physician Information

To make the referral process as simple as possible, we have provided our patient referral form to enable you to download, complete and fax to our office, as well as a complete list of the insurances we currently accept. We also have some informative Barker Sleep brochures available upon request to best prepare your patients for a visit to our facility.
