New Patient Information

If you are a new patient at the Barker Sleep Institute, we welcome you! Once you schedule your initial consultation with us, you will receive New Patient Information Forms in the mail. Please fill these forms out completely and bring them with on your first visit. You will also need your medications, insurance card and driver’s license. We are proud to be able to accept most all insurances, and have listed them below for your convenience. If you do not see your provider listed, or if you have any questions regarding your appointment, please call our office at (865) 584-3850 and we will be glad to help.
